مانیتورینگ عمق بیهوشی (Bispectral Index) یک سیستم مانیتورینگ نوروفیزیولوژیک مدرن است که به طور
پیوسته منحنی الکتروانسفالوگرام بیمار را در طول مدت بیهوشی عمومی آنالیز می کند تا بدین وسیله بتوان
سطح هوشیاری و آگاهی بیمار را مورد مطالعه قرار داد.
به بیان ساده تر،سیستم مانیتورینگ عمق بیهوشی  روشی است برای تعیین عمق بیهوشی بیمار.
طبق گزارشات عنوان شده، امروزه استفاده از این روش در اکثر جراحی هایی که در آن ملزم به بیهوشی بیمار
هستند، استفاده می شود.
مقادیر مشخصی جهت تعیین پارامترهای مورد نظر در معرفی این تکنولوژی آورده شده است.
محدوده ی تعریف شده اعداد 0 تا 99 را شامل می شود، بدین صورت که عدد صفر بیانگر وضعیتی است که در
آن EEG فرد کاملاً آرام است (Silence) .
اعداد نزدیک به صد معرف حالت هوشیاری (Fully Awake) کامل است و اعداد مابین 40 تا 60 شاخصی برای
نشان دادن بیهوشی طبیعی است که میزان تغییرات این اعداد در بین تولید کنندگان این گونه تجهیزات، متفاوت
است.

تعیین الگوهای مشخص مانیتورینگ عمق بیهوشی

تعیین الگوهای مشخص بیهوشی عمومی برای افراد نرمال در یک محدوده ی سنی تقریباً مشابه ( به عنوان
مثال بچه ها با سن بالاتر از یک سال )، این امکان را برای متخصص بیهوشی فراهم می آورد تا بتوان میزان
داروی مشخصی را جهت ثابت نگه داشتن عمق بیهوشی بیمار و کنترل شدت بیهوشی مشخص کند.
در نتیجه فرد بیمار پس از انجام عمل جراحی به علت تزریق دوز مناسبی از دارو، سریع تر و به طرز مناسبی به
هوش خواهد آمد.
تعاریف جدیدی در زمینه ی پیشرفت مانیتورینگ عمق بیهوشی در حال انجام است که امید است در آینده ی
نزدیک به قابلیت های این سیستم اضافه شود.
در صورت تحقق این اهداف ،مانبتورینگ عمق بیهوشی باعث کاهش وقوع هوشیاری های با عوامل درونی (
درونی بدنی ) در طول فرآیندهای جراحی با ریسک بالا می شود، همچنین نقش مکملی را در پیش گوئی مدت
زمان به هوش آمدن (Recovery) خواهد داشت، مخصوصاً در زمانی که بیمار دچار آسیب های شدید مغزی
شده باشد.

معرفی سیستم مانیتورینگ عمق بیهوشی  به بخش های مراقبت ویژه، این امکان را برای پزشکان فراهم می
آورد تا دوز استانداردی از ماده ی بیهوشی را برای بیمار تجویز کنند.
همچنین از آن برای مانیتور کردن وضعیت بیمار ( به همراه اندازه گیری فشار داخل جمجمه ) در درمان
مشکلاتی موسوم به Burst suppression استفاده می شود.
هم اکنون از مانیتورینگ عمق بیهوشی در جابجایی بیماران با وضعیت حاد در آمبولانس و هلی کوپتر نیز
استفاده می شود.

domain, frequency domain, and high order spectral subparameters.

BIS monitor

The BIS monitor provides a single dimensionless number, which ranges from 0 (equivalent to EEG
silence) to 100.
A BIS value between 40 and 60 indicates an appropriate level for general anesthesia, as
recommended by the manufacturer.
The BIS monitor thus gives the anesthetist an indication of how “deep” under anesthesia the patient
is.
The essence of BIS is to take a complex signal (the EEG), analyse it, and process the result into a
single number.
Several other systems claim to be able to perform the same thing.
This calculation is very computer-intensive. The recent availability of cheap, fast computer
processors has enabled great advances in this field.

EEG

When a subject is awake, the cerebral cortex is very active, and the EEG reflects vigorous activity.
When asleep or under general anesthesia, the pattern of activity changes.
Overall, there is a change from higher-frequency signals to lower-frequency signals (which can be
shown by Fourier analysis), and there is a tendency for signal correlation from different parts of the
cortex to become more random.
The developers of the BIS monitor collected many (around 1000) EEG records from healthy adult
volunteers at specific clinically important end points and hypnotic drug concentrations.
They then fitted bispectral and power spectral variables in a multivariate statistical model to produce
the BIS index.
 As with other types of EEG analysis, the calculation algorithm that the BIS monitor uses is
proprietary.
Therefore, although the principles of BIS and other monitors are well known, the exact method used
in each case is not.

History

The BIS was introduced by Aspect Medical Systems, Inc. in 1994 as a novel measure of the level
of consciousness by algorithmic analysis of a patient’s electroencephalogramduring general
anesthesia. This is used in conjunction with other physiologic monitoring such
as electromyography to estimate the depth of anesthesia in order to minimize the possibility of
intraoperative awareness.
The US Food and Drug Administration (FDA) cleared BIS monitoring in 1996 for assessing the
hypnotic effects of general anesthetics and sedatives.
The FDA further stated in 2003 that “…A reduction in awareness provides a public health benefit, in
that BIS technology can now provide anesthestists with a way to reduce this often debilitating, yet
preventable medical error”. Aspect Medical was acquired by Covidien in 2009.

Relevance

The BIS is an electroencephalogram-derived multivariant scale that correlates with the metabolic
rate of glucose.
 Both loss of consciousness and awakening from anesthesia are correlated with this scale.
The efficacy of BIS index monitoring is not without controversy Some controlled studies have found
that using the BIS reduced the incidence of memory but this was not confirmed in several very large
multicenter studies on awareness A Cochrane review in 2014found that.
“Four studies (7761 patients) that used clinical signs as a guide to anaesthetic administration in
standard practice, as the control group, demonstrated a significant reduction in the risk of awareness
with BIS monitoring.
Four studies (26,530 patients) compared BIS monitoring with end tidal anaesthetic gas (ETAG)
monitoring as a guide to management of anaesthesia and they did not demonstrate any difference in
terms of intraoperative awareness”.

Sociedad de Anestesiología Reanimación

The Sociedad de Anestesiología Reanimacióny Terapéutica del Dolor de Madrid recommends
monitoring of anesthetic depth in accordance with literature-based evidence. BIS, however, is not
explicitly endorsed.
In fact, they cite an American Society of Anesthesiologists (ASA) statement saying that the decision
for cerebral function monitoring should be made on an individual basis.
